Notice and Disclosure

In connection with your entry into your clinical training program (Master of Science in Counseling Psychology degree program), you will be required to obtain a consumer report as part of the process. In the event that information from the report is utilized in whole or in part in making an adverse decision with regard to your clinical practice, before making the adverse decision, you will be provided with a copy of the consumer report and a description in writing of your rights under the federal Fair Credit Reporting Act.

The Fair Credit Reporting Act gives you specific rights in dealing with consumer reporting agencies. You will be given a summary of these rights together with your report.

By signing and submitting your application for admission, you hereby authorize Avila University, through Validity Screening Solutions, to obtain a consumer report and/or an investigative report as required by your educational institution of application and/or attendance. The consumer report may include, but not be limited to, criminal history, employment verification, education and/or certification verification and other healthcare-related information repositories. Your personal credit history will not be retrieved.

DISCLOSURE REGARDING BACKGROUND INVESTIGATION
AVILA UNIVERSITY (“the Institution”) may obtain information about you in connection with your entry into the clinical training program, from a third party consumer reporting agency. Thus, you may be the subject of a “consumer report” and/or an “investigative consumer report” which may include information about your character, general reputation, personal characteristics, and/or mode of living, and which can involve personal interviews with sources such as your neighbors, friends, or associates. These reports may contain information regarding your criminal history, social security number validation, motor vehicle records (“driving records”), verification of your education or employment history, or other background checks. You have the right, upon written request made within a reasonable time, to request whether a consumer report has been requested and compiled about you and disclosure of the nature and scope of any investigative consumer report and to request a copy of your report. Please be advised that the nature and scope of the most common form of investigative consumer report obtained with regard to entry into the clinical training program is an investigation into your education, criminal, and/or employment history conducted by Validity Screening Solutions, PO Box 25406, Overland Park, KS 66225-5406, 866.915.0792, www.validityscreening.com, or another outside organization. The scope of this notice and authorization is all-encompassing, however, allowing the Institution to obtain from any outside organization all manner of consumer reports and investigative consumer reports now and throughout the course of your enrollment to the extent permitted by law. As a result, you should carefully consider whether to exercise your right to request disclosure of the nature and scope of any investigative consumer report.
